Entering & At A Show

  • AFRMA Exhibitor/Seller’s Form – ALL exhibitors/sellers must submit this form prior to exhibiting or selling at AFRMA events (use as many pages/forms as needed) (PDF; form interactive so you can fill in then send as an e-mail attachment in the Adobe Acrobat PDF program – pic of envelope on top Send file as email attachment, or save the PDF file after you’ve filled in all the information and go to your e-mail and attach that saved file to an e-mail and send to us; if your Internet browser does not use Acrobat to view PDFs or support forms, download Adobe Acrobat to your computer, open the form in your browser and save the form, then open the form in Acrobat on your computer and fill out and send using the above instructions)
  • AFRMA EVAL Entry Blank – entry blank for evaluations (EVAL Day by appointment) for Members (page 1) or Non-Members (page 2) and the back side to both (page 3) (PDF; form interactive so can fill in before printing)
  • How to fill out an AFRMA Entry Blank – Notes and samples on filling out an AFRMA Entry blank using the PDF Entry Blanks as samples (PDF).

  • Supply Order Form – Order your supplies for the next show (PDF; view in Acrobat)
  • Booth Space Form (interactive PDF) – We have non-animal commercial booth spaces for rent at AFRMA Shows (i.e. if you have craft items, are a distributor for a product, etc.). You will get one 6′ or 8′ table for your booth if available. Items must not be the same as AFRMA sales items. You must be signed up to sell and paid for by the show deadline (MONDAY prior to the show; PayPal for payment). Fill out the Booth Space form and e-mail to the Show Sec. on the Show Flyer, then pay with PayPal on the Show or Pet Entry Blank form (bottom of page). If your Internet browser does not use Acrobat to view PDFs or support forms, download Adobe Acrobat to your computer, open the form in your browser and save the form, then open the form in Acrobat on your computer and fill out and send to the e-mail address).
